MEDICAL EQUIPMENT RENTAL CONTRACT

hereinafter referred to as „the Parties“, hereby enter into this Medical Equipment Rental Agreement (the „Rental Agreement“), whereby the Parties agree as follows:
I. SUBJECT AND TERM OF THE RENTAL AGREEMENT
1. Lessor shall provide the following Medical Equipment to Lessee for temporary use: ................................... (the „Medical Equipment“).
2.1. The General Terms and Conditions for Site Use and Medical Equipment Rental, („GTC“) published at www.grijaplus.bg are an integral part of this Rental Agreement.
2.2. A copy of the GTC shall be handed over to the Tenant upon signing of the Rental Agreement and the Tenant shall sign a Declaration of Acceptance of the GTC.
3. This Lease Agreement is entered into for a term of ________ days/weeks/months from ..... to ....
4. In the event that the Tenant wishes to extend the term of the Rental Agreement, the Tenant shall notify the Landlord in writing at the following e-mail address: info@grijaplus.com or by telephone at least 1 day before the expiry of the term.
5. Within 1 day of the notice, the Landlord shall notify the Tenant in writing of its decision.
II. RENTAL PRICE
1. The Tenant undertakes to pay the Landlord a rental price of ________

3. Extension of the term of this Lease shall be valid upon written consent of Landlord and payment in advance of the rental price for the new term.
III. TRANSFER AND RETURN OF MEDICAL EQUIPMENT
1. The medical equipment, together with the instructions for use, shall be handed over to the Lessee at the time of signing the Rental Agreement, which in this part shall serve as an acceptance and handover protocol, by which the parties certify the proper condition of the equipment and its handover to the Lessee.
2. The medical equipment will be used by the Hirer / a person from his household at the following address: ..................
3. The Lessee shall arrange for the return of the Medical Equipment at its own expense to the following address ................... on the day following the expiration of the Rental Agreement or the termination of the Rental Agreement in the other cases provided for below, as well as in the event of an exercised right of withdrawal.
4. Upon return of the Medical Equipment, a report shall be drawn up in the manner and procedure specified in the GI, certifying its condition.
III. TERMINATION OF THE RENTAL AGREEMENT
1. The lease agreement shall be terminated at the expiry of the term or by mutual agreement expressed in writing.
2. The Landlord has the right to terminate the Rental Agreement unilaterally upon:
default by the Lessee;
Misuse of Medical Equipment;
non-payment or late payment of amounts due.
IV. FINAL PROVISIONS
1. For the purposes of this Rental Agreement, the definitions in the GTC shall apply.
2. The provisions of the GTC and the Bulgarian legislation in force shall apply to the matters not covered by the contract.
3. All disputes between the parties shall be settled by mutual agreement, failing which by the competent court.
This Contract shall be executed in two counterparts, one for each of the Parties.
